A list of all templates usable with SmartSimple's Census Integration variables is available for download from the U.S. Census Bureau's website here:
In order to use the variables from the Excel file above, take the "Unique ID" from column C, add an underscore as the 7th character, and add "E" at the end, so that B01001004 (Sex by Age - 5 to 9 years) becomes B01001_004E
A csv-formatted version is also downloadable from SmartSimple here, that contains the correctly-formatted variable in the left-most column:
